Clusterflux, from here makes awesome kydex. I'm just not sure he does it on order.

Seems like I remember people recommending Scott and Christina Gere, I think they make some sheaths for Spyderco? I think the email was scgere@AOL.com. I understand they live near Spyderco.

The sheaths from Gere are Ok. The only place I get kydex from anymore is Invictus Kydex. Matt is a true craftsman and he gives well executed and well thought out options with his sheaths as both a knife holder and a deployment platform.

I'll second the Clusterflux kydex option.....fantastic work at more than reasonable prices.....Cluster has done a lot and I mean a lot of work for me and some of my students.....nay a complaint!....He does especially nice work and is a joy to work with......I highly recommend him for your kydex work......you can PM him here right on the forum. (psst for what its worth the new kydex with the carbon fiber imprint (forget the name) is awesome, not only great looking it is very very resilient. Ask Cluster about it!

There's a member on BladeForums called "GLOCKCRAZ" who has made a custom kydex sheath for one of my Mules. He does nice work.
