Is ball-lock failure common?

I got a Poliwog last spring with a very rounded tang, it was pretty much a slip joint. The tang was not contacting at the bottom of the ball. It was only going part way and squeezing the ball out. So I took a diamond file to it, flattened out the tang until I couldn't get it to fail anymore.
It's very solid now, and the lock only has a tiny bit of extra travel.

I may have to do this. Even with the lint out of there it's not infallible.
