Any one work on forklifts?

When I was at my last job I would help out the mechanic/technician whenever I could. They were pretty good at keeping up with maintenance but sometimes things would get overlocked. This is the worst thing I saw. It's a pin for the steering rod end. Not sure how someone managed to use it like that.
