Please enter me in the contest! Here are my responses...

4. Varies. At least $500.00, sometimes more.
5. Depends on the instructor. Maybe $150.00 to $200.00 for a "big name" with a good curriculum.
6. Depends on the material and instructor. Cross country for a really special class, but generally a couple of states.
