My vote goes to the new Swayback; at least to me it looks upside down. The end of the handle sweeps up instead of down, the end of the blade sweeps down instead of up and the lines appear to run opposite of where they should. This might qualify? Techno1 with incorrect CPM-S30V blade instead of the correct CTS-XHP blade. Spydiewiki alleges that a single run of these was mistakenly made and sold. Photo lacks sufficient detail but the blade is marked “CPM-S30V”

Does a gift you gave to someone else count? As is the custom I gave this dagger to my wife when we got married. Gold, coral, & abalone inlaid spiders, [that’s how it relates to this forum:)] flowers, Spider web, etc.; silver fittings, all on bone scabbard and hilt. Hand forged Edo period blade

Figured out how to post pics! 1st one: Mini-probe with shackle #0008. Whoops! Just found out this is actually the full sized probe! Ruler measures to 6 inches. The mini probe only measures to 5 Inches. Not too much info on these or the pre-diver out there. Spydiewiki doesn’t have an entry for these ...
